Vet diagnosis and
treatment limit amount *
I have read the Vet Care Release form and authorize Dogwalker Etc to obtain emergency medical services in the event I cannot be reached. I will be fully responsible for all fees and charges and will pay for all charges incurred on my behalf upon my return.
Dogwalker Etc! dogwalker, etc.

In the event that any of my pets become ill, injured, or at significant risk of experiencing a medical problem (at the start of service or while in the care of dogwalker etc. sitters), I give permission to dogwalker etc. to seek veterinary service from a veterinarian or veterinary clinic. My preferred veterinary services are listed on each individual Pet Information Disclosure. Other veterinarians or emergency care clinics chosed by the pet sitter are acceptable.

I ask dogwalker etc. sitters to inform the attending clinic or veterinarian of my requested total diagnosis and treatment limit of $enter amount below per pet / all pets (most common values are $200, $1000, or unlimited). I understand that efforts will be made to contact me regarding any treatments, illness, injury, or potential problems as soon as the condition is deemed not life threatening and/or contact is possible. I understand that dogwalker etc. sitters work hard to prevent accidents and injuries, and that such problems may occur no matter how well a pet is cared for. I agree to allow dogwalker etc. sitters to use their best judgment in handling these situations, and I understand that dogwalker etc. and its staff assume no responsibility for the actions and decisions of the veterinary staff, the health, or death of my pet(s).

I will assume full responsibility for the payment and/or reimbursement for any and all veterinary services rendered, including but not limited to diagnosis, treatment, grooming, medical supplies, and boarding. Such payments will be made within 14 days of the initial incident. I also agree to be responsible for all Special Service fees assessed by dogwalker etc. sitters for emergency transportation, care, supervision, or hiring of emergency caregivers, and will pay such fees within 14 days of each incident.

I further authorize dogwalker etc. sitters and my primary veterinarian(s) to share all of the medical records of all of my animals with veterinary clinics in an emergency in the interest of providing the best care for my ill or injured animal(s).

Every dog, cat or other pet type at the site of service will be current (per my veterinarian's recommendations) on its vaccinations prior to the arrival of any caregiver. I will also make arrangements to guarantee that each animal will remain current on its vaccinations throughout each service visit period.

I agree to notify dogwalker etc. of any signs of injury or possible illness before any visit as soon as the condition appears. Dogwalker etc. reserves the right to cancel service at any location where a pet with a potentially infectious condition exists. Dogwalker etc. strives to provide clean, safe service to each of our clients. In doing so, dogwalker etc. strongly recommends that each pet and large animal be vaccinated, dewormed, and protected from harmful insects according to veterinarian recommended standards.

I wish the following action to be taken in case of emergency:

New Clients Introduction

You'll need to have the following items ready during your initial interview:

  1. A key (or some way we may enter your home for visits i.e. garage door opener). We will provide a key-chain and a code (no address will be put on your key).
  2. A 2nd key that we can code with permanent ink (we can make this complementary spare).
  3. If your sitter has to return at a later time to retrieve your key, additional charges may apply.
  4. Emergency contact information for yourself, and 2 other contacts recommended.
  5. A list if items you plan to leave out during pet sitter visits (such as paper towel, medicines, etc).
  6. Trip information (if applicable), including Hotel and if you plan to have visitors while away.
  7. Your initial consultation should last between 30min ~ 60min

Guidelines

How to Prepare for your interview with your Pet Sitter:

  1. Set aside 30-60 min for your interview.
  2. Have your pets home & introduce them to the sitter during your interview.
  3. Tell the pet sitter about all health problems the pets may have—major or minor.
  4. Walk your sitter through the home & pet(s) daily routine. (show food bowls, leashes, litter boxes, extra supplies, etc)
  5. If a dog is not accustomed to walking on a leash, request that they practice a short walk with her/him before they leave.
  6. Let your sitter know of any house "quirks" (leaky sink, stuck window, etc)
  7. Show your sitter where your breaker panel is located.
  8. If your pet sitter will be providing services inside your home, provide a house key at the time of the interview (garage door openers/apt fob if applicable).
  9. If you have new keys made, try them out to make sure they work.

How to Prepare Your Home before services begin:

  1. Clean food and water dishes before leaving town. Provide extra food, litter and other supplies just in case you're not able to return when planned.
  2. Clean out the refrigerator to avoid smelly food spoilage and wash the dishes in the sink to prevent ants, flies or other pests.
  3. Adjust the thermostat to keep the house comfortable for the pet during your absence. In warm weather, a closed-up house can get dangerously hot in a short time.
  4. Leave a list of phone numbers in case the pet sitter needs to contact service or maintenance companies while you are away; i.e.plumber, electrician, cleaning service, yard maintenance, pool service, etc.
  5. Tell the pet sitter of any household areas that are off-limits to your pets or to the pet sitter. Secure access to those areas before leaving home.
  6. Make a list of pet's favorite hiding places. This will prevent the pet sitter from worrying if pets are not where expected—the sitter will know where to look.
  7. Make sure your dog's collar fits—Otherwise he may slip out of a too-large collar while being walked.
  8. If a dog growls or snaps at the pet sitter during a "get-acquainted visit," understand that the pet sitter may ask that the owner make other arrangements for the dog's care during any absence.
